Existing patients may request an appointment online. New patients should call 206 623-2225 to schedule with our receptionist.
Please fill out new patient intake forms if you are a new patient. If you were injured in a motor vehicle accident, kindly will fill out our auto accident intake.
Please arrive 15 minutes before your scheduled appointment if you have not already filled out our new patient intake forms
Kindly let us know if you are running late or need to cancel your appointment. We appreciate that sometime “life happens,” so we do not charge late fees or cancellation fees.
New patients visits typically last about an hour. Read about what to expect on your first visit here.
Devine Chiropractic participates in many insurance programs and accepts workers’ comp, auto accident, Personal Injury Protection, Medicare, and all PPO plans. Most insurance policies cover chiropractic care and other services offered at our office. If you have little or no chiropractic insurance coverage or no insurance at all, we will work with you and discuss payment arrangements. Please call 206-623-2225 for more information

While we are happy to verify insurance information and coverage from your insurance company and file your claims as a convenience to you, it’s always best to verify this information with your insurance company directly by calling the 1-800 number on the back of your insurance card.
